This research paper reports that the secretion of the aforementioned snails stimulate fibroblast proliferation and rearrangement of the dinamic structures of cells (actin cytoskeleton) and it has superoxide dismutase (SOD) and glutathione-S-transferase (GST) antioxidant activities as well as other multiple modes of antioxidant action, acting at the level of free radical production and also sequestering free radicals.

It also stimulates extracellular matrix assembly and the regulation of enzymatic metalloproteinase activities, which limits the extent of the damage during wounding and scar formation.

GAGs are complex sugar molecules that participate in the regulation of physiological processes through their interactions with a wide variety of proteins and proteoglycans. They also play a critical role in cell proliferation, migration and adhesion. The mucin not only contains those molecules but, what is far more important, it stimulates the proliferation of glycosaminoglycans within the skin matrix.

GAGs are of utmost importance for the prevention and treatment of stretch marks.

The reason is GAGs have a large water holding capacity, occupy a large space in the extra-cellular matrix and fill most of the intercellular space between collagen & elastin proteins. They perform a critical role as shock absorbents and provide binding, hydrating and swelling pressure to cell tissues. In doing so, they enable skin to withstand compression forces and thus prevent tearing and scarring of the deep layers during pregnancy, puberty growth spurts, overstretching when practicing body building (in association with steroids) or by more than average weight gain.

Proteoglycans (PGs) are proteins linked to glucose chains (GAGs) and are vital components of connective tissues. They play an important role in modulating the structure and regulating the functions of cells and molecules within the skin. The timely turnover of ProteoGlycans influences the development and differentiation of cells.

Wound healing depends on the level of GAGs and PGs which if not adequate leads to abnormal scars. They are thus critical for healing and prevention of stria distensae / stretch marks (atrophic scars) keloid and hypertrophic scars.

Scars and stretch marks develop as a result of disruption of the integrity of the dermis, which is the deeper layer of skin. The appearance of scars and stretch marks is due to bands of collagen which the body lays to form strong bonds in order to hold the skin together where it was once cut, as seen in surgery and trauma.   Stretch marks are similar to scars in that there is pulling of the skin, leading to small linear scars, but there is no actual separation of the skin as with true scar formation. 

Stretch marks appear after the skin is stretched excessively such as in pregnancy, spurt of growth during puberty / adolescense, rapid weight gain, and physical activity especially with weight lifting. Pathological endocrine conditions like Cushing's Syndrome may favor the appearance of stretch marks. Some drugs, mostly systemic cortisone and progesterone, are other factors contributing to the development of stretch marks.

Many different forms of treatment have been tried to improve the appearance of stretch marks.

Topical creams , such as those containing Vitamin A (retinoic acid), glycolic acid or moisturizers such as squalene, olive oil derivatives, emu oil, rose hip oil, cocoa butter, aloe vera butter or shea butter have been used to prevent stretch marks. But when used to treat stretch marks, improvement, if any, is very slight. Some found a little improvement, especially in early stretch marks, whereas others found no difference.

Dermabrasion with a diamond fraise, if performed superficially, gives no improvement. If performed more deeply, it can cause dyschromia , scarring or even rupture of the skin at the depth of stretch marks. Dyschromia is the technical name for irregular or patchy discoloration of the skin that can be caused either by variations in pigment density (melanin), changes in blood vessels (vascular changes), or growth of foreign organisms on the skin.

Microdermabrasion by itself, tends to give irregular results. It is uncomfortable and scarring may occur.

Lasers especially pulsed dye laser, have shown some improvement, mainly on red stretch marks.

Superficial peel can be induced by 20–70% glycolic acid. The experience of glycolic acid treatment of stretch marks is disappointing. This is not surprising as alpha-hydroxy acid acts superficially, mainly by diminishing the adhesion of the stratum corneum cells.

Medium depth peels with standard Trichloracetic acid (TCA) at 35% to 50% (mass per volume) have been tried with irregular results and complications where large areas have been treated.

Deep peels with phenol are too dangerous for extrafacial skin, because of their potential for life threatening complications, local scarring and persistent hypochromia (decrease in the amount of hemoglobin found in red blood cells).

The following classification of stretch marks based on external signs can be useful to assess treatment responses.

Stage I

Fresh, inflammatory usually discolored marks.

Stage II a

White, superficial marks without laddering and without palpable depression at the surface of the skin.

Stage II b

White, superficial marks without laddering but with palpable depression at the surface of the skin.

Stage III a

White, atrophic striae with laddering measuring less than 1 cm width, without deep pearliness.

Stage III b

White, atrophic striae with laddering measuring less than 1 cm width, with deep pearliness.

Stage IV

White, atrophic striae with laddering measuring more than 1 cm width, with or without deep pearliness.

 

Stretch Marks Treated by a Novel Treatment - Two Options

Home Microdermabrasion

No skin preparation prior to the exfoliation is necessary. No pre-peel treatment with any cream or any other regime is necessary. First, clean your skin with any alcohol cleaner or with a glycolic acid cleanser. Then just massage the BIOSKINEXFOL™ CREAM on clean skin, with either the tips of your fingers or with a dry and soft cloth. Simply apply a small amount to the skin and rub in a rotational pattern. Wipe after.

For practical purposes, the area to be treated may be divided into squares of 10 × 10 cm (4 × 4 inches). The goal of the microdermabrasion is to remove the superficial layer of the epidermis and allow the cream to penetrate evenly. The microdermabrasion may be performed until you start to feel the first pain of abrasion. When you touch the surface of the abraded skin, you will feel roughness of the skin, as you are touching keratinocytes, which are better hydrated than the surface corneocytes.

Keratinocytes originate in the basal layer from the division of keratinocyte stem cells. They are pushed up through the layers of the epidermis, undergoing gradual differentiation until they reach the stratum corneum where they form a layer of dead, flattened, highly keratinized cells called squamous cells.

This layer forms an effective barrier to the entry of foreign matter and infectious agents into the body and minimises moisture loss. Keratinocytes are shed and replaced continuously from the stratum corneum. The time of transit from basal layer to shedding is approximately one month.

Corneocytes are cells derived from keratinocytes during the late stages of terminal differentiation of squamous epithelia. Corneocyte desquamation at the skin surface is a complex biologic event which is normally regulated for providing an inconspicuous shedding of single corneocytes.

Application of the home microdermabrasion cream and occlusion of the area.

The quantity to be applied is about 0.5 g per 10 × 10 cm area.

The cream will be the active factor at the depth of the stretch marks, while also acting on the normal dermis between the stretch marks. Both elements will create the improvement. The microdermabrasion ultra-exfoliation stimulates the fibroblastic reaction of the normal dermal cells located between the stretch marks.

The cream will be absorbed deep within the skin during the microdermabrasion massage and works fine just like that, but it works best beneath occlusion.

Use occlusion: either taping or a nonpermeable membrane, or kitchen plastic wrap secured with bandages, or an occluding substance —-usually a cheap petrolatum cream or petroleum jelly.

The occluding dressing holds perspiration close to the skin, hydrating the top layer of the epidermis, the stratum corneum. Topical substances are absorbed into a moist stratum corneum 10-100 times more efficiently than with dry skin. A common method of occlusion is to apply plastic wrap to the affected area and secure it on all sides with tape. Occlusive dressings should be left on for at least 2 hours and often are applied at bedtime so as not to interfere with daytime activities. Because occluded areas remain moist, they are at risk of developing bacterial infections. In general, occlusion should not be used for more than 3 days in a row.

When occlusion is applied the cream a) continues for long its enzymatic digestion of both the damaged proteins and the coagulated proteins that form when skin is injured as it is with the microdermabrasion cream and b) continues for a longer period of time the stimulation of new collagen and glycosaminoglycan formation.

Otherwise absorption may not be enough, due to the barrier of coagulated proteins. Also to avoid the desiccation of the area by air contact it is best to apply a dressing or sheet on top of the cream to create an occlusion.

Removing the occlusion

The occlusion should be left for about 6 hours. The duration of occlusion is a factor that influences the efficacy of the treatment. The longer the occlusion the stronger is the action.

Microdermabrasion or acid peelings alone do not yield benefits. In our technique, the benefit comes from the association of two or the three elements: microdermabrasion, cream absorption, and occlusion.

Multiple treatments are required to improve stretch marks.

Treatments should be repeated every 2-3 days. Superficial stretch marks will require a 2 months treatment to achieve about 50% to 80% improvement, whereas it may take as much as four months of treatments to significantly improve old, large and deep stretch marks.

General Guideline:

4 - 8 Weeks - for mild or minor scars or marks.

8 to 12 Weeks - for noticeable results for old and deep stretch marks.

Results are better and fewer treatments are needed with more recent and/or superficial stretch marks. It works as well for stretch marks seen during puberty or many years after pregnancy.

Before starting the treatment you should know that the final result of the appearance of your stretch marks will be an improvement but never a complete disappearance. So you have to make a final decision when to stop the treatment.

Complications

The only complication that may need management is transient hyperpigmentation usually seen around 4 weeks post-microdermabrasion, especially on darker skin types. This hyperpigmentation can be resolved with the application of a 4% hydroquinone bleaching cream twice daily.

Advantages

This form of treatment offers many advantages and stays within a safe zone. The presumed mechanisms of action are reviewed below.

The microdermabrasion is performed just deeply enough to remove the corneocytes. These cells are responsible for the impermeability of the epidermis. Removing this layer allows the cream to penetrate.

The goal of the cream is to stimulate the fibroblasts of the healthy zones between stretch marks and act also at the depth of the stretch marks.

The glycoprotein enzyme and the moisturizing and lipid enhancers contained in the cream start a partial dissociation of the corneocytes and enhance the penetration of the product by diminishing the superficial tension. Breaking of the barrier function of the epidermis creates an immediate stimulation of the lipids, DNA and growth factors into the keratinocytes that will stimulate their differentiation.

After the action of the abrasion and the cream on the healthy parts of the skin between the stretch marks, the cream also acts at the depth of the marks, stimulating re-epithelization and enhancing fibroblast production.

The aim of the procedure is to allow the cream to touch the desired level of the skin and, because of the occlusion, to remain in contact long enough with the dermis at the depth of the stretch marks, where the cream can exert its various beneficial effects on collagen synthesis and so improve the marks.

Conclusion

A new home microdermabrasion cream containing enzymes that dissolve scar tissues and trigger new biosynthesis of collagen and healthy glycosaminoglycans from within the deep layers of the skin is described which is effective in improving the cosmetic appearance of stretch marks. Neither skin type, nor localization, nor class of stretch marks limits this treatment.

Microdermabrasion is relatively superficial but enough to break the skin barrier to allow deep penetration of the ingredients in the cream.
